How your leveling request is handled
This is an independent service-request website. When you send a request, we keep the contact details, location, observations and consent you entered. Vercel provides private storage; GoHighLevel holds the lead and its follow-up notes. The symptom selections can carry into the form during your browser session. Contact fields are not kept in browser storage by this form.
Keeping and sharing the record
Requests stay on file for follow-up and recordkeeping, with no scheduled automatic deletion. Call (385) 503-3119 for privacy questions or to request access, a correction or deletion. We check a provider before sharing information under the consent you give with your request.
Hosting and your browser
Vercel may process technical request information to serve and protect the site. The form keeps its entries on screen if sending fails. It does not add them to URL parameters.
